<p>You don’t have to. Most people don’t. I did because there were also teacher comments on my Q1 report card, and I thought that they would help me in admissions. </p>
<p>If you do, just write a cover sheet with your app info (Name, HS, B-day, address, etc.), and mail it to the admissions office.</p>
