A question about REC letters

I am applying to at least 5 schools in the fall. How do I ask for recommendation letters? I am asking my calculus teacher for a recommendation letter but how do I ask for letters for the 4 other schools? Do the teachers actually write 5 different letters or can you use the same letter for each college?

Also, how do I approach my teachers? Do I just give them 5 applications and ask them to follow the directions for each recommendation letter? I don't understand this at all

thank you for answering

Yes, your teacher will write one LOR and attach to the individual college’s forms usually. He or she has done this (hopefully) dozens of times.

Firstly, inquire if he/she would feel comfortable writing one for you. Then inform that five colleges will need them. Then supply the schools’ forms along with stamped envelopes (as a courtesy). Then a nice thank you note and/or small gift.
