A teacher messed up the name of a summer program that I went to...

She gave me a copy of the rec letter, I read it, and instead of X summer program she put Y which is held at one of the schools I'm applying to. (I applied to it and asked her to write the rec letter but I didn't get in, which is probably why she got them mixed up.)

So now one of my teacher recs is inconsistent with the rest of my application. What should I do? The choices that come to mind is to apologize for making my information confusing and asking her if she could clarify the name issue (and provide envelopes + stamps, of course, if it needs to be mailed which I'm guessing it is), or writing the colleges myself...?

Help please!

If you signed the statement saying that you waive your right to see your rec, then this seems like the best solution. Your teacher will likely realize her mistake once you mention it and will be more than willing to make the correction.

If you didn't sign the waiver, either method will work. I don't think it's a big problem, it's a mistake that's easy to make since she knew you applied to program Y.
