Academic Suspension Appeal Letter (Need help revising)

Good afternoon Dean _______ ,

 I am writing to appeal my academic suspension resulting from my failure to make academic progress during the 2015-2016 school year. I am asking to be re-admitted for Fall 2016. I understood what was required of me when I was placed on academic probation. Last semester, spring 2016, I recorded a grade point average below a 1.0, prompting my dismissal from the University.
  Looking back on my freshman year I felt like I rushed into college not fully understanding the extent of my new responsibilities. I had a decent first semester at 2.54 and I felt that I could do well going forward. With my new found confidence in my independence, I became become distracted by several experiences that college offers and recorded a 1.0, placing me on academic probation. That summer leading into my sophomore year I began to take on a series of mental and financial burdens and placed unnecessary stress on myself. Instead of seeking help I allowed my pride to get in the way and rushed back into school without improving my mental situation. I began to carelessly skip class, spent most of my days in the gym, and partied too much. Since then I have taken measures to better my situation. 
  In the past I never really applied myself at all and I plan to fix that. I will better utilize syllabus week to both understand the class’s workload and get to know my professors. I am willing to meet with professors and classmates outside of class so that I can get help to better understand the material. I want to attend outside lectures and complete extra credit assignments to achieve the highest possible grade. I am dedicated to designate 20 or more hours a week to my studies. I am going meet with a counselor at CAPS once every two weeks to help monitor and maintain my mental focus. I am also eager to meet with you, and your secretary at least once a week to discuss my academic progress and to request your help with my career aspirations going forward. I have realized that I would like to become a Psychotherapist , and a B.A degree in psychology will put me in a great position to accomplish that. I am aware of the commitment this degree calls for, however I am determined to be successful. Being reinstated for the Fall 2016 semester will put me at a great advantage to achieve this goal.
