Accidentally OMITTED info on application!

I'm looking at a summary of my college application that has already been submitted. It says that the "applicant does not have the discretion of omitting any information of their academic history". There is a section that says, "Country of secondary school" and another one that says, "Your age while attending"...
I COULD NOT find anywhere to fill out this information during the online application. If this option was there, it was most likely hidden or something.
I don't want to be banned for committing information... I had no idea!

It just says the name of my secondary school and the month/year of graduation.

Can anyone please help? The college I applied to is CUNY.
Should I contact them and let them know?

I would contact them directly.

Correction: I just recalled the fact that there was a section that said, “have you attended only one secondary school?” and I selected “yes”. This could be why it didn’t let me choose the years I attended and country of secondary school, but it’s just a hunch. I’m still not sure.
