ACT Extended Time application?

Hi,
I'm interested in gaining some insight for my application for ACT extended time.
Two years ago, I was diagnosed with ADD, and although it was a minor case, it did negatively affect my academic potential. I've been trying several different medications over the years, as prescribed by my doctor, but none seem to work exactly the way I need it to. I have not been receiving extra testing time in school because my philosophy was that in the real world, no one gets extra time. I worked harder than others had to in order to get grades and scores that were satisfying, which were oftentimes less than my peers (but still within the A-B range). I've already taken the ACT before, and scored a 29 (33 E, 29 M, 24 R, 30 S). Most of my friends and peers that I have been in classes with over the years are now getting scores much higher than mine, and I feel like if I'm able to get the extra time I need, I will be up to par with them.
I've also heard that the ACT does not give extra time very readily. Although my case is less severe than others, it is very real and I am very determined to get the extra time I need. If anyone could give me any advice on my application at all, it would be extremely helpful and I would be very grateful.
