Act Extended Time??

Hello,
I am trying to apply to ACT to get extra time for my testing. I do have ADHD and testing anxiety and I have a print out of my diagnosis with the DSMIV codes. I also have lupus and I get a lot of "brain fog". I am on a crap ton of meds too along with prednisone which can cause psychiatric issues. I also have been diagnosed with generalized anxiety disorder(since 8th grade). I have been seeing my psychiatrist since 9th grade (im a senior). I also have documented learning issues with reading since first grade.
As if that werent bad enough I also recently saw my neurologist who told me I have a part of my vision missing in the peripheral view. I have had a 504 plan with my school since 10th grade and I do require special testing at school. Usually I will go in the counseling office if I cant finish a test on time in class (it can take me up to 2 to 3 hours to finish a test). Sometimes I will stay an hour and a half past class to finish my math test. Its not that I dont know the material, I just have trouble accessing it. Its like I will look at my test and I will be reading a passage and then I notice I dont have much time and after that its just like the words evaporate out of my head as soon as I am done reading them. Its very frustrating.
The first time I tested (last march) I got a 20 because I also had a sinus and ear infection on top of ADHD. This last time that I took it (april) I got a 24 but I still struggled immensely at finishing the test on time.
It is very hard for me to write timed essays (even in school) because I have tremors from the medication and my hand cramps as a result. I try to write through the cramping but it looks like chicken scratch.

I was just wondering if anyone thinks that I meet the criteria or if I even have a shot at getting extended time on the ACT.

Definitely. A lot of my friends have ADHD and got extra time from ACT no problem.
