<p>It’s not supposed to act as a transcript, the admissions office just likes having things in generally the same format - it makes the applications easier to read. Put down whatever classes you want to (I think I put down Orchestra and P.E.) - but know if you don’t, they will still appear on your transcript, so it’s totally fine if you decide not to.</p>
