if I were to submit an additional letter of reference from a university professor whose course I am taking through a dual enrollment program, should I use Common App's teacher's reference forms, or is that exclusively reserved for my high school teachers?

Given that it is supplemental, I think that it could be done either way. You could use the common app form for structure but if the professor has a preferred way for writing a reference, I think that would also be acceptable.

which one is better? She’s fine with both

The content, not the format is what’s important.

There are instructions on the Harvard site for submitting an additional letter of recommendation.

PhotoOp - Here is what I found on the Harvard site but it does not deal with format.

You’re right - the info I had is not specifically for supplementary recommendations - but for General Guidelines:

“Please label your materials with your legal name, address, high school, and date of birth.”
