Additional Recommendation?

I'm applying to Brown University and I personally know one of the history professors there. He's the organist/music director at my church and I've known him for 7 years due to being in the choir. I asked him to write a recommendation for me and told him to just mail it to the admissions office since there wasn't an option to submit additional recs in common app. Is it ok if he just writes the recommendation without any of the typical recommendation forms that teachers fill out? Is it ok if I did that?

i think you can submit it through common app. I did it too. you fill out some info about the professor and then the system will send an invitation to the professor to ask him upload the recommendation letter. this works for me!

Yes, it is OK to have letters of rec sent by mail if not done through common app or on forms. The key is that the letter needs enough on it to identify you and get it to your file. Your name is usually enough but adding at least high school identity can help particularly if you have a name that could easily be the same as others (such as John Smith).
