<p>I’m applying online and have just submitted part 1, but I need to send them an “Admissions Application Signature Form”. They say I can include my check or fee waiver (I already paid with credit card, so I’ll disregard this) and a picture (I’ll upload one onto my online application (part 2) later, so I’ll disregard this too. What do I need to do exactly? Do I just write my name on it, sign it a few times and send it to them? Should I put the return address on the envelope too? Can I type out the Applicant name, DoB, SSN (as opposed to write it in pen) and then sign the rest of the form in pen? Thanks!</p>
<p>Tell you what... For me... The whole thing was absolutely ridiculous. I had print-previewed my application in pdf format before and I saw this number on the top right hand corner of each page and I happily assumed it to be the applicant ID no. assigned to me (since i'm international). I printed that signature form out, filled in my assumed-ID no., mailed it over and the next day, I received an e-mail from Penn informing me of my actual ID number. Fantastic... Fortunately, there is the DOB column, so I guess they'll still be able to match it to my file. What I did was to print that form out, fill in the stuff by pen and signed it a few times. I don't think it really matters whether you typed the other stuff before signing or whether you used your pen for everything. So long as it's legible, everything will be alright.</p>
<p>So when I mail this in, all I have to have in the envelope is a single piece of paper (the form), since the photo and money order/check is not required of me? The website did not specify, but I'm guessing I only have to mail in the form.</p>

<p>yes it is an appropiate time. i dont think u need to use a courier at this time. there's no need to rush. using the courier now will be a waste of money.</p>

<p>i uploaded the picture and mailed in the resume because there was space for picture online but not for resume. i believe either is fine though.</p>
