Alternate Majors Form

Hello all,

I applied EA and was denied to my first choice major in the College of Engineering, However, Purdue has offered me the option to choose an alternate option for admission. I can choose up to three majors. I am having trouble finding the alternate major form…has anybody been able to find it and if so where? Thanks in advance.
