<p>Yeah, there's a Part II for both the school report and the mid-year report. It's 1 page, double sided.</p>
<p>I looked at it. It's just some extra random ratings from your counselor. I don't think its required based on the website.</p>
<p>My counseler didn't do the rating sheet at all. If I recall correctly, it's just a rating list with checkboxes and a request for a rec. My school has its own college forms for teacher and GC recs. My GC just sent in a general college rec for me, and that was it.</p>
