My teachers do not know English. What is
the proper way for filling out the Teacher Evaluation Form in this case?
Should I translate the questions on the Teacher Evaluation Form for her, and
then translate her answers into English and type them into the Form? After
that, my English teacher would check and confirm the accurateness of
translation by writing on the translated form.

As for the Recommendation, my teacher writes it in the native language and
then I am going to translate it and give it to my English teacher to check
and sign that the translation was correct.

Is the process correct, or what should I do differently?
Should I send forms in both the original language and in English, even if my
English teacher signs on them that the translation is accurate?
