<p>There is supposed to be a parent info card and another reserve card that you are supposed to fill out saying whether you intend on attending or not and neither was with my packet. I have a stamped envelope but netiher card. Anyone else encounter this problem?</p>
<p>My daughter has both that you are missing. It was in the pocket on the last page of the booklet with the envelope --- right in front of the "Visit" book.</p>
