<p>I think this is almost certainly part of the common app (it is a part of the school report). Your counselor should attach the letter to that part of the application, or mail it in separately.</p>

<p>You should use the form provided. If this is a serious problem, for some reason, then a standalone letter works too, but the form is better if you can do it.</p>

<p>You can send in the letters before the rest of the app, but if you can submit the first part early, all the better. In any case, your materials will not be lost. Things come out of order all the time.</p>

<p>Do whichever one you think can speak more informatively about your abilities and potential. Usually, the one who knows you better is better, but make sure your statistics teacher mentions that you are taking/have taken calculus and are doing well.</p>
