<p>It won't let me submit my application and is asking for my:</p>
<p>"Permanent Address Valid: From Month (mm)"</p>
<p>"Permanent Address Valid: From Day (dd)"</p>
<p>"Permanent Address Valid: From Year (yyyy)"</p>
<p>What do I put for these if I was born in the US and am a US citizen?</p>
<p>Bump...I need to know - it won't let me submit until I fill this out.</p>
<p>A date far in the past to a date far into the future.</p>
<p>For mine, I just put from my birthday until sometime in 2011... i beleive there's somewhere that (roughly) says if you don't plan on leaving michigan just put a date at least 2 yrs in the future.</p>
<p>I just left the "to _ _ _" part blank and it accepted my app</p>
