<p>On form 7, the Transcript Request Form, I fill that out myself, right? There doesn't seem anything there for my counselor or teacher to fill out. I've asked this before, but got no response. I'll probably have more questions later too.</p>

<p>Nickel Xenon- I'll try again. This form is filled out by YOU. It is a VERY simple form and is very easily filled out. You don't mail it to USC. You give it to your school so they will send your transcript to USC. Your school should then send this form with your transcript to USC. You said your counselor at your school is not helpful, but there is someone at your school who sends out transcripts. Ask around. Ask the principal. Give that form, filled out by you, to someone at your school and ask them to send your transcript to USC. You can fill out a stamped addressed envelope made out to USC when you give this to your school. You don't have physical access to your official transcript, so it must be sent by your high school. </p>

<p>This form is not used if your school has a different procedure for sending the form like if you have to request your transcript online or with a different form that your school uses. In that case, follow your high school's procedure for sending transcripts and you don't have to use this form.</p>

<p>My counselors and the people in the office don't mail things for you. They'll fill it out for you and you yourself have to mail anything that needs to be mailed. My counselor, for my counselor evaluations, said she's too busy to mail them and said I have to do it myself. Do they have to do anything to the envelope? Like, sign or stamp the back? Becuase my counselor said to make sure my teachers signed the back of the envelope for my teacher evaluations, which I have to mail myself too.</p>

<p>Do we submit Form 5 even if we already submitted our scores through collegeboard?</p>

<p>If you already submitted SAT scores from CB, you don't have to do part 5. If you already had your high school send your official transcript, you don't have to do part 7.</p>

<p>Oh, I go to a public school, but I don't mind sending the forms myself. I'd prefer it actually. The people in the counseling office are horribly unreliable. With 2,400 students, you're never priority for them. It takes a week before they get back to you after making an appointment.</p>

<p>Edit: You're supposed to request transcripts and they give them to you sealed with the school profile in the envelope with the transcript. Then you can mail them off yourself. But I really don't think my school mails them themselves. If they did, it'd take them a loooong time to mail them off rather than me taking it to the post office.</p>

<p>All public schools in Dallas with 2400 students have a Registrar Office that will send transcripts to colleges- that's their job. You don't have to make an appointment.</p>

<p>If you really want to send it yourself, just make sure you are sending a sealed Official Transcript from the high school Registrar Office. You cannot just put it in your own envelope and seal it and call it official.</p>

<p>Oh, it's already in an evelope that's stamped on the back. Is that alright? The envelope is one of those official school ones that has the address and everything on it. But is it okay to put that into a bigger envelope with my form?</p>

<p>Edit: The counseling office takes care of all college stuff. We probabl do have a Registrar, but they're probably also in charge of something else too and busy. My school is bad about this kind of thing, mostly because the counselors are underqualified. But thanks for all the help you're giving me. I really appreciate it.</p>

<p>Thanks. I asked the lady for a transcript and she asked if it was for mailing. I said yes. All she did was put the transcript into an envelope and fold it with the school profile, seal the envelope, stamp it, and told me bye. I was kind of, "Okay...what do I do now?"</p>
