<p>i think that you need to use either all jhu or all common app. it might look a bit sloppy/disorganized if you mixed and matched.</p>

<p>In answer to your question, you may mix and match parts of the JHU app. and common app. when it comes to teacher rec forms and misc. items. The only time they request you don't mix and match is with the primary application forms -- either apply with the JHU app. Part I and II, or apply with Common App. and the Common App. supplement.</p>
<p>My school supplies teachers with their own standardized teacher recommendation form. Both my teachers used that. Guidance also has a standardized sheet for the school district. The actual forms for the teachers that hopkins uses say something like "you can send all of this on a different sheet if you want." So I wouldn't worry.</p>
