Just realised that these three forms look exactly the same except that they have different names. Are we actually required to submit all three of them?

They are not the same. The "School Report" is the first one that goes with your initial transcript in the fall. The "Midyear Report" is the one that goes with your midyear transcript (after first semester/second quarter). Your GC fills it out only if there's something that's changed from the first one. The "Final Report" is the one that goes to your chosen college with your final transcript and proof of graduation. The GC again fills it out if something else has changed from initial/midyear report.

Note that you do not send these. Your GC/school does.

Am I right to say if nothing has changed, say after mid-year, I will not have to submit the final report?

And is my counsellor supposed to write different testimonials for these three forms?

No, you're not right. First, you do not submit the final report; your GC does. It serves as a cover letter to your final transcript and proof of graduation. It should contain all the same identifying information as the other reports your GC sent, so that your final transcript can be filed properly. (Of course, this only goes to the school you have decided to attend.)

No, your GC doesn't have to write different testimonials. The initial one is sufficient unless something has changed.

I see. Thank you!
