My daughters club coach is asking for a recruiting profile with her academic and athletic info as well as a pictures and sport stats. If you have one you would be OK sharing could you PM me?
If you want a standard template for your sport, just go sign up for a free profile at one of the recruiting services. We did the freebie at NCSA just to get a feel for what coaches needed to know, but it’s also a convenient place to direct anyone who wants to know more about your athlete. Be advised they will be calling and emailing you a bunch to move up to the pay service, but we were easily able to resist the “opportunity”. 
One other thing we had to do on our own was assemble video of skills and games. We just put those up on Youtube with a first name, school, year and club name in the description so that people who know what they’re looking for can find it and those that don’t just see another recruiting tape.
Full Name, Address, Phone number, Students Cell, Parents Name, High School. GPA, Test Scores, Coaches Names and number or email. Position if a team sport. If individual sport, listing events and outcomes. There is no set information. But to give the person an overview how to contact you, your scores to see if you will succeed at the college and then of course your “talent” to see if you can fit in or not. Most interested coaches will ask you to fill out their college recruitment form and that will list all of the information they will want in addition to all the information already provided.

Most schools also have a form linked from each team’s web page that asks sport-specific questions about the what the coach wants to know about potential recruits. Glance at a few of those to gain another perspective.
