Berkeley School Forms

OK so I am working on my application to UCBerkeley (I always spell that wrong :-( ). And I can't find the forms that I give to a school personnel to fill out with my transcript etc. Is this all done by me and then checked by UCB once my application is in?

Short answer: Yes, it’s all done by you…no forms from your school.

I assume you’re out of state or international? The UC system is a little different to navigate. Basically, all they want is your application and offical test scores from SAT or ACT…and nothing else! So that’s why you’re not seeing anything about “forms” for your school to submit. See, e.g., [Prospective Freshman FAQs](http://students.berkeley.edu/admissions/general.asp?id=2107&navid=N)

However, that doesn’t mean you don’t need your school’s help. You’ll need assistance in reporting your coursework and grades to meet the eligibility requirements. Have you seen the calculator at http://www.ucop.edu:8080/eligibilitycalc/results.jsp?startOver=ff</a> ?

Bottom line, you self-report everything–no forms from your school are required until after you are admitted, when an official transcript is required to confirm your self-report. Hopefully, your counselor is up on what the school needs to provide to you for filling out the application. I suggest you talk with your counselor ASAP to confirm. The kids at our out of state school had a deadline of Nov. 8 to meet with their counselor about UC apps just to give the school enough lead time to process everything to meet the Nov. 30 deadline considering Thanksgiving holidays.

Good luck!
