<p>My school developed its own Counselor Report Form to send in place of the CommonApp Form. Our form does not have a question about discipline and counselors do not provide that information.</p>
<p>EDIT: Sorry, did not even realize how old this thread was. I don’t know how I came across it- didn’t mean to bring up an old issue.</p>
