As you probably know, getting into the PLME program is so selective, that it it is very hard to predict any student’s chances of getting in. After 20+ years of interviewing primarily for PLME, and having students I know very well apply, I can tell you some thoughts:
I was underwhelmed with your odds, compared to the students I have seen get in, re your board scores, and likely class rank. (are you in the top 5 in your class?) BUT your research time , and hospital time is a very critical positive for you. I would consider retaking the SATs if you think you can get math up above 750.
Definitely get a reference letter from your research director (MD preferable, if not then PhD) if you think they will write you a good one. I hope you are continuing that senior year too.
Being from Ohio is likely a plus, being Asian background is not. Water polo is a nice quirk/plus. (Is that on a serious basis, are you still playing senior year?) Depth of activities is often more important than multiple “short” or less involved ones.
Biggest question is why do you want to go to Brown specifically for PLME? (and why not early decision, if so?) Do you want to go to Brown if not into PLME? (I think you have a good chance at regular admission). Admissions will be looking for a sense of the answers to those questions even if you don’t specifically answer them.

Thanks for responding. I don’t want to apply early decision because I want to see how the total cost of tuition will be at other places that I apply to.

If I understand correctly, my chances of getting are very slim due to my low math subject test score and test scores in general.
