<p>Say that I my teacher gave me back the common app evaluation forms. I make copies of it and mail them to colleges. Yes, I do have recommendation letter from these same teachers. But I want to vary both recommendation letters and evaluations. Can I reuse the same evaluation that my teacher has filled out? Or does she or he have to redo it the same form for each individual college?
By the way, what's the difference in submitting an evaluation form and recommendation form? Which one do colleges prefer?</p>
