Can someone explain how letters of recommendation work?

<p>Do the teachers just like go online and fill it out? Or do they have to mail something? I just want like a general explanation of how this works.</p>

<p>Also, how should I ask my teachers for letters of recommendation? What should I say? Do I need to give them anything?</p>
