<p>I think that's what my guidance counselor did (not positive) because she mentioned copying the form for as many schools as I had to fill it out for each one. I'm concerned because her rec letter is sealed in with my senior grades, so I'll never be able to check for sure unless I ask her. But my main question is - does it matter? I mean all it is is showing the grades, unless each school has specific senior grade forms (which, I don't know yet since I've only made it through the common app one). Additionally, when are the midyear reports and final reports due? When I printed out my senior grade form, three sets of forms came out - the semester report, the midyear report, and the final. I'm going deadline insane, and I just want to make sure all my ducks are in their rows.</p>
<p>Son and school used the same form no matter what the school was or what form they really asked for. </p>
