Can't understand the difference

I'm an international applicant following the A-Level system. I'm facing difficulty in understanding the difference between these CommonApp forms:

(a) Midyear Report (MR)
(b) Final Report (FR)
(c) Secondary School Report (SR)
(d) International Supplement (IS)

What exactly do these forms contain? How do each of them differ? When are they due? Which ones should I submit if I'm applying ED (Early Decision)?

Is the counselor evaluation a TE (Teacher Evaluation) form or is it included with the SR (Secondary School Report) and FR (Final Report)? I also see that all three forms (SR, MR and FR) have short spaces for evaluation. Will the counselor write the same evaluation in all 3 spaces?

It would be grateful if my questions are answered.

(a) Midyear Report - Your transcript (or report card) for the fall semester of senior year; sent at around December of January
(b) Final Report - Your final transcript at the end of the year (you send it after you get accepted)
(c) SSR - report by the your school that includes information about class size, rigor of courses available, grading policy, etc
(d) IS - not sure, since I’m not international, but it’s probably just an extra form for YOU to fill out

If you apply ED, you will submit your app, Secondary School Report, guidance counselor evaluation, teacher evaluations, and International Supplement. You will submit the Midyear Report once your Fall semester final grades are released (you may already have gotten accepted ED to a college), and submit the final report at the end of the school year when all your grades are finalized.

Thanks a lot. So all my grades for the 9th, 10th, 11th and 12th go into SSR, right?
