Chance an anxious, very ill playwright?

As a quick preface, I am very ill junior-going-on-senior with a rather critical illness that has impaired me quite a bit--both physically and cognitively. I've been in and out of hospitals and it's been rough to get through school--but I'm trying pretty hard to make it through next year.

So, here we go with stats and ECs:
(I'm new to this website, so tell me if I'm doing anything terribly wrong)

SAT:
Math - 660 (I know, I know--math is my weakness)
CR: 750
Writing: 760
GPA: Somewhere along the lines of 3.85ish unweighted, and they (meaning the guidance department at school) never told me my weighted grade--probably somewhere along 4.3-4.5? I haven't a clue. But my grades were severely impacted by the illness.
APs: AP Language and Composition, AP Music Theory, AP Studio Art (All for junior year)
IB: N/A
Awards: These are kind of telling as to who I am...ahh well. Let's just say I've won a lot of playwriting competitions, including making the finals (top 4%) in a national competition, as well as awards for that play's technical set and costume design in a statewide theater festival.

Extracurriculars/Volunteering:

-Junior year independent study – I wrote the first student-written play to ever be put on at my high school, also assistant directed, designed the costumes and the set, and composed the music
- It is likely that this play will also soon be performed off-Broadway in New York City, which is really exciting
- I've written a lot of plays, including a full-score musical (so music composition as well)
- Art school program
- Tutoring blind children--I read Braille
- I'm trying to get a book published soon--it's coming along
- Volunteer theater set design for the local middle schools

So what do you say? I'm pretty sure I don't have much of a chance, but I figured I'd just post to see what others say. I'm quite nervous for the college application process next year.

Really, REALLY nervous.

I don’t know, I wouldn’t say you don’t have a chance at all. In fact, while you’re certainly not a shoe-in, I think you have a decent chance.

The big question for adcoms will probably be whether your illness will impair your ability to perform at Yale. Also, I got in with a 620 M, so don’t worry about that. As long as you’re above a certain threshold I don’t think test scores mater tooo much–at least they won’t hurt you. Then again, if you think you can get a higher score, do it.

Thank you very much for your feedback :)
