Definitely need to bump your Sat score up 120 points or so for Cornell and most of the UC’s, and michigan, but the others will be fine.

I’m in a very small private high school(no more than 40 in my class) with limited honor/AP class. Cant help with my gpa anymore(UW 4.0), and I believe my math&science course rigorousness are ok: linear algebra&cal bc(11), AP Stat(12), AP physics(10), AP Chem(11), AP Env Sci(12)

For test scores, since I have already taken SAT for multiple sittings, do I need to take ACT in Dec? I believe it would be 32-34. Will it increase my chance?

I think it won’t hurt to take the ACT on December.

So take it, and replace your SAT score.
