I am sure my recommendation will be great. I am working as a teacher assistant (leadership position?) for Art History and my Art History teacher will surely write a great recommendation about my passion and dedication that I have put so far.
Same with my counselor.
Major: Art History w/ Pre-med
So what are my chances for Vassar College? And for extra curricular activities that I have done for only a year, is it even worth mentioning on college app?

I think with things as they stand right now you will be denied. With GPA and ACT considered, the 25% line is 3.64 GPA and 31 ACT. Even right at the 25% line, you typically need a big hook to help you get in. More realistic if you are at the 50% line, and that is 3.76 GPA and 33 on ACT.
It is not unreasonable for you to apply though and you do have a slim chance, but Vassar is a reach for you for sure.
I think you would get in. I’m a white female, my ACT score was only a 30, and my AP exam score grades were far from stellar (APES 3, APUSH 4, AP World 4, AP Lang & Comp 3). My GPA was high though; I was in the top 2% of my graduating class and I was involved in extracurriculars. I applied ED, and I think that definitely helped. If you write a really thoughtful common app essay, good supplementals, and get good teacher recs, I don’t see why you can’t get in. You definitely have a good chance.
Vassar is definitely a reach for you. I would say take the ACT again if you’re up for it, and really work hard on your essay. Vassar really values excellent writing. Also, if there are any optional responses on the application be sure to do those!
