Any tips/advice would help as well.

I’d say it’s a high match/low reach for you.
Above average for UCLA: weighted GPA, ACT score, amount of AP classes, and class ranking
Average for UCLA: unweighted GPA, consider taking SAT subject tests in two or three of your AP class subjects?
Below average for UCLA: SAT score and your extracurriculars. You have little community service hours and no variety/meaning(assuming since you did not elaborate) in it either. You are also not in any clubs/organizations. Have you had a job perhaps?

“The average admitted applicant to UCLA for the Fall Quarter 2013 had a weighted GPA (a GPA that includes all extra grade points for honors or AP coursework) of 4.41, an unweighted GPA (no extra points) of 3.89, an SAT Reasoning Test score of 2055, an ACT Assessment composite score of 30, 21 semesters of honors/AP course work completed between 10th and 12th grades, and 53 semesters of college prep course work overall.”-http://www.admissions.ucla.edu/faq/FR_Not_Adm.htm

UCLA: high match(in-state)
low reach(OOS)

where are you from? Try to boost your SAT to a 2000+. THat can boost chances.

I think you can just use your ACT scores for application.
