Your scores and grades would put you in the ballpark, but with Yale’s low admission rate, every application is a reach. Since you mentioned your health, I would suggest you seriously speak to your medical team to see if the high stress environment of Yale would be conducive to your overall health. Also, will there be medical personnel familiar with your medical history to monitor your health and any emergencies which may be beyond the capacity of the Yale Health Center. I don’t know how far you live from New Haven, but kids can panic when they are sick and far from home - even for more minor conditions. You may want to take that into consideration.
If you are accepted, Yale will do everything possible to accommodate you but only you, your family and your medical team can determine if it is a good way to go.
