I am planning to apply to CS this Spring. I was really sick from June until now and all of my performances were affected. Do I have chance to get into CS major if I have 3.5 in M126, 3.1 in Phys121 and 3.8 in CSE143 (UW GPA). My Cal I, Cal II, CSE142 were fine because I were healthy at that time (They're in the 3.8-4.0 range but they're transferred credits from CC). My concern is M126 and Phys121.

May I know the average GPA of prequisites of someone who get accepted?

One of my friends had a 3.8, and another had about the same (he received a 4.0 in every class I took with him). One friend of mine didn’t get in, and he had about a 3.0. These are all cumulative. It’s EXTREMELY competitive so make sure you make up for anything with your essay.

Specifically the part on misconceptions. About 40% of people who try for CSE get in, which isn’t very competitive at all - “…students in the top of our applicant pool (who we have space to admit) typically have grades between 3.3 and 4.0 overall. We specifically look for grades close to 3.5 in math, science, CSE and English; these subjects are important indicators of success in upper-division CSE courses.”

The only thing that will stop you from getting in is if you are somehow a terrible fit for CSE and it shows in your application essay. Otherwise stop worrying brah.
