<p>Directly on the website they had listed these requirements and provided no direct link for the form. The Lehigh requirements are as follow:</p>
<p>Application for admission to the program is made through Lehigh Office of Admissions. Admission is based on SAT scores (a minimum combined score of 1350 Math and Critical Reading), ranking in the top 10% of high school class, and demonstrated maturity and motivation for dental school. Application deadline is January 1.</p>
<p>It is possible that Lehigh may send the form once they think you are qualified for the program, so I would not stress since they did not designate a link on their website for the form. :)</p>

like on the NYU thing it says that in your NYU application just check that you want to do the 7 year program but could you do it using the common app too?</p>
<p>You can call the school and ask this question they will help you. I would always try anyway. Sometimes your non academic credentials and experiences may be viewed more valuable than just top grades. Dental schools look for lots of shadowing and experience.</p>
