Chances for ivy leagues and other top university's such as northwestern

I am a rising senior, and I wanted to know how well my chances at any of the IVY league universities, and other top universities, such as Northwestern and Stanford were.
Sat Scores: Math-790, Writing-790, Reading-660 (I know that is a little low, I am going to retake in October and hopefully bring it up to a 720 or higher)
SAT Subject Tests: Math 2-780, U.S History-740 Chemistry-700
GPA: Unweighted-3.9, Weighted-4.6
Additional Info: I am an IB student and have 4 HL classes instead of the standard 3. I also have an AA degree from my local community college. I have also taken 6 AP classes and will be taking 4 more during the school. I have passed all of them, and will most likely pass all of the rest however, I did get a 3 in two of them.
EC's: I am a member of 4 clubs, and I am treasurer in 2 of them, and VP in another. I have been a member of all of them for at least 3 years. I played 2 years of soccer for my school as well.
Volunteer Work: I have 500 hours of community service hours. 350 of which is at a hospital. (I want to go into medicine).
I am NOT a URM.
I am basically wondering what my chances are for any of the IVY League schools and if so, how important would my college essay be in affecting my chances. Also, ideally, I would love to be accepted into Northwestern's HPME program, so if any one knows about that I would love any opinions on that as well. I do not have any research experience though, mainly due to cost and lack of availability in my area. I do have over 150 hours of physician shadowing though.
Any input would be greatly appreciated.

You definitely need to bring up the CR score to 700+ for these schools.

^Agreed

Assuming you do that, you’ll have a very strong chance at any school :)
