I would assume you would be listed as a competitive applicant given the fact that you had to balance a part time job as well as competing in two varsity level soccer all the while maintaining a rigorous high school schedule. The GPA and ACT puts you range for consideration, but admissions considers a wide variety of sources and examines the intricacies of your socioeconomic background, educational resource availability, among many other elements and attributes that I do not know of.

I say your extremely well rounded in terms of extracurriculars, with two leadership positions and significant volunteering experience at a hospital as well as real world job experience.

I'd say you have a above average to an exceptional chance at admissions given if projected figures you indicate match or exceed your estimations.

Good luck in applying, Its great to see quality applicants considering Johns Hopkins university.

I don't know what happened in your freshman year...a D in Algebra 1?? But your improvements will probably have a very positive effect on admissions. I would not call those EC's less than stellar at all...in fact they are quite good. It takes a tremendous amount of time to play a varsity sport and being Captain at 2 sports is no easy feat.

Also community service hours are good, and work experience is only a plus. A good essay will increase your chances even more.

I say you have a good shot, especially @ ED.
