Rank 4/909 (I have a big High School)
I've been top 5 in my class throughout all of High School

I started a club at my school and am the current president -raised over $1000 last year for local charity.
I am a senator in my schools NHS chapter
I play on the schools varsity lacrosse team for 3 years (club level)
I have 200+ service hours

So what does everyone think??

I’d say it is a low-mid reach, if you send your ACT score. Your rank and schedule are both good, but your ACT is average and you won’t get in because of your ECs. It’s probably too late to do anything spectacular with ECs, but if you can get your test scores up, you will have a fighting chance.

If i were to just submit my ACT, would that help, or am i better off submitting everything

I think you have a really good chance. Just work hard on your essay (make it UNIQUE) and your college application. Also, do NOT submit your SAT score. Just submit your ACT score. It may seem low, but it is in the mid-range for trinity students [Duke University Admissions: Class of 2013 Profile](http://admissions.duke.edu/jump/applying/who_2013profile.html). However, keep in mind, that the class of 2014 was the hardest to ever get in, so don’t take it for granted. Good luck!
