1) I want to get into a Direct MD program, especially Rice Baylor. What can help me stick out? I'm taking a total of 6 APs at high school (overall) and about 7 math and science courses at local uni's.

2) My brother wants to be an engineer. He has the opportunity to take Calculus BC as a freshman or pre-calc as a freshman and BC sophomore year. If he does BC freshman year, he can take more physics and math courses at local uni's before graduating. Is it worth it to take BC as a freshman? At his current pace, he will have BC as a sophomore but if he does pre-calc locally during the summer he will be placed in BC as a freshman. I don't want to push him too much since the pre-calc class is 5 hours a day for the whole summer including saturdays.

BC is like college frosh calculus; engineering majors typically take that and one more year of math. So either way, he’ll likely finish all of the math needed for engineering while still in high school. So there is no need to rush to take BC as a high school frosh.

Even if he decides to major in math, BC as a high school sophomore would allow him to take plenty of college level math while still in high school, assuming he can get to a nearby community college or university as a high school junior, and a nearby university with a good math department as a high school senior.
