CMU's Own teacher recommendation form or Common APP's??

Son is going to approach teachers this week about writing letters of recommendation. The Carnegie supplement has a special form for teacher recs. Is it better to use this or the generic one from the common application? Does CMU prefer their own? IT seems to differ only slightly. THis is our oldest child so the process is new! It's rather confusing!

I don't think it matters. My son gave his teacher's ONE common ap teachers recommendation and then told them which schools would take it. (And he also gave them stamped envelopes for the schools.) He figured it would be less work for them to be able to photocopy the common ap form.
