<p>who should fill out the college official's report. im assuming its a dean, but what if your dean doesnt know you that well? i go to a large state school. is it okay to have your advisor fill it out?</p>
<p>Just take it to the deans office. someone there will fill it out. They get lots of these forms. Colleges just want to know you are in good academic standing.</p>

<p>If your Adviser is willing to write a rec/mini evaluation for you (and some Dean forms do seem to have space for that), it would make sense for you to take that (or have your adviser forward that) to the Dean. The Dean can append it to the Dean form which, as menloparkmom says, is more to state that you are in good academic and behavioral standing at the school.</p>
