OP, my heart goes out to you. I would suggest your D apply everywhere, write about her Math Disability in her essay, and then when she gets to school, they can help her. My sons both struggled in math, and my oldest actually had to take a Remedial Math class at his college (George Mason) before he was allowed to take any other math class. My son at VCU is in the Art School, so he can get by with taking “Art Math”. Otherwise, he would also be in remedial math.
Just FYI, VCU DOES accept Clep College Algebra credit, it corresponds with their Math 141. Here is the link for your viewing:

@sbgal2011 - yes, that is the plan. We are visiting all schools and she will apply to all in-state (VA) and likely a few OOS – Appalachian State appears to have a great support program so that jumped on to our list too.
So - scores for D20’s second ACT diagnostic are in…she increased her math by 2 pts to 19!! Increased other sections all by 1 pt. New diagnostic ACT composite score of 23, up 1 pt from previous diagnostic. If we can creep up to 26 I think that would be a good goal.
So…am following up on this thread I started…D20 has taken the actual ACT twice now and she did quite well! On the first run…English: 32 Reading: 31 Math: 17 Science: 25. Then on the second she raised her Math score to 21 and Reading to 34. Her first composite score was 26, the second composite was 27 and if superscored, her composite is 28! She also took the SAT three times…I know, probably overkill. Still waiting on June SAT scores, but from other tests her highest math was 540 and reading/writing was 670 - superscore of 1210.
I am thinking that submitting her ACT scores would be best route – they are comparatively better, yes?
https://collegereadiness.collegeboard.org/pdf/guide-2018-act-sat-concordance.pdf gives these concordances:
27 ACT = 1260-1290 SAT
28 ACT = 1300-1320 SAT
21 ACT math = 530 SAT math
32+34 = 66 ACT E+R = 710 SAT EBRW
