Common App Teacher Eval Forms

If a teacher writes a rec letter for you, do you have to send in the Teacher Eval forms from the common app that have the check-off boxes and all that. Can you just send in the rec letter?

Does anyone’s teacher actually check off straight rows of Top few ever encountered? Just wondering here.

<p>Yeah, they want the check boxes filled. At least that's the pattern I noticed (most schools said that it was okay for the teachers to attach a letter of rec, but they should also fill in the boxes).</p>
