<p>I'm a bit confused about which forms you need to send to your current college and past high school to fill out. There are 3 forms: Final report, midterm report, and registar report. It says on the common app that "The Registrar Report in this section can be used if you need to submit transcripts from more than one college." Does this mean it's optional? Also do I just send a copy of the final report to both my college and my high school or JUST to my college? Finally, how do I send my high school transcript via the common app? Thanks.</p>

<p>You send the Registrar report to your current college to fill out for the colleges to which you’re applying for transfer. You’ll have to contact your former high school to send your transcript. They’ll know how to do it.</p>
<p>So who fills out the final report?</p>
<p>The Final report is for your HS GC, it is for your final HS grades/rank/etc. and evaluation. You need to check with your HS to see if they send the transcript with the report, separately or electronically.</p>
<p>The Midterm report is for you to take around to instructors about midway through sp sem for your unofficial grade at the time.</p>
<p>The Registrars report is to be filled out by your college to show that you are in good standing. Colleges vary in who fills out this form, it could be the Registrar, your academic advisor, Head of Major Dept., etc.</p>
<p>I have called several Universities. Many don’t need the Final Report, they just need your HS transcript.</p>
<p>Yes, not all colleges require the Final report, it varies with the school, so applicants must always check the college website for details of what to send. The CA is a general application designed to cover all possibilities for the schools that use it, so not all forms will be applicable to all schools.</p>
<p>I’m pretty sure it’s the College Advisor/registrar that does the final report. These questions indicate that it has to be your college: </p>
<p>“Have there been any changes in the applicant’s disciplinary status at your school since you submitted the original School Report?”</p>
<p>“To your knowledge, have there been any changes to the applicant’s criminal history since you submitted the original School Report?”</p>
<p>It is the registrar who submits the “original School report”, thus the question must be directed to the registrar. Since the registrar is at your college institution, the form cannot be for the HS GC. The only thing schools want from your high school is the high school transcript. The HS transcript is only sent to the schools that you are accepted to. That’s why it accompanies the high school transcript.</p>
<p>^No, the high school fills out the Final Report. It asks for the attached grades from 11th and 12th grades.</p>

<p>The reason it’s called the Final Report is because it includes final HS gpa/rank/etc. as opposed to the SSR form which is for submitted before sr year is over (for fr admissions).</p>

<p>Perhaps for UCs if transfers work like fr admissions, but many schools require the HS transcript w or w/out the Final report at the time of application. A few random examples:</p>

<p>Hmmm. You are correct. I’m applying to Yale, Harvard, and UPenn. All of their websites say they require the HS transcript. Good to know. Sorry for the confusion. Thanks and good luck.</p>

<p>As a student applying for transfer to multiple schools, I assume you would photocopy the mid-year report to send to schools, as well as have the guidance counselor do the same with the final report, correct?</p>
