Common App's "School Official's Report" -- what?

<p>On the transfer common app, there's a school form called the "school official's report." Half of it looks like it should be filled out by my academic advisor (the questions about "how would you assess this student," etc), and the other half looks like it's aimed at the Dean of Students (whether I'm in good academic standing, the school seal, etc). </p>

<p>Who do I ask to fill out this form?</p>

<p>Oftentimes, both. I had to go to three separate offices to get my report completed. Kind of a hassle, but it usually doesn’t take long to fill out.</p>

<p>I had to go to four different offices. Registrar, Dean of Students, Advisor, Judicial Affairs.</p>
