CommonApp: Advisor's or Dean's Rec?

Hello, I need your help.

I am using the CommonApp to file my applications. While the schools I am applying to list a "Dean's Recommendation" as a requirement on their individual websites, the CommonApp asks me to fill out my Advisor's Information and then plugs that into the "College Official's Report," instead (found under "School Forms").

Does it not matter that schools say they want a Dean's Recommendation, and should I go ahead and ask my Advisor to fill the form instead? I have seen my Advisor a total of two or three times, she barely knows me. I have never met the Dean and I had a big one credit lecture-hall-pass/fail style class with the Assistant Dean (+ another professor) -- so she wouldn't have a clue who I am, either.

Still, does the app better present itself when the College Official's Report is filled by the Dean? What should I do?
Should I fill the "Advisor" fields of the CommonApp with my Dean's Info? Er...

Thank you for your help!

I printed out a blank version of the sheet, because my advisor doesn’t have the information I need. I’ll just fill the info in manually. It’s fine to ask an advisor though if they have the necessary info.

Hey arcadefire1027, thanks for your answer. What do you mean, necessary info? What’s that – just your academic records?

Pretty much. My advisor said he doesn’t have access to class rank or anything.

College Official’s Report requires access to both academic and disciplinary record, so you may have to get two signatures on the form. In my college, that form is filled out by the student’s counselor (academic portion) and by the dean of students (disciplinary portion).

I took it to my adviser. My adviser had never met me before I asked her to fill out the form. Everything worked out just fine.

arcadefire1027, drek’thar, college_ruled, thank you for your answers! :)

And if I need the advisor to fill out five copies of the College Official’s Report, should I print five copies, sign each one, and give them to her to fill out, or instead sign one copy, and she can photocopy the rest?

(Can my signature be photocopied?)

Also, if I want to e-mail my professor my signed College Instructor Evaluation, can I include a pdf stamp of my signature (earlier scanned)? I won’t be able to meet with her personally but don’t want to mail things physically – extra work both for me and her.

Thanks!
