Has anyone been admitted to graduate school on a conditional basis?

My friend just told me she was accepted to her master's program on a conditional basis. She satisfied all the requirements of admission: above a 3.0, above 500 on both the v&m gre and 4.5 on the writing and a BA from an accredited school. Her gre and gpa were not stellar.

Her only requirement is to get B's(3.0) on 3 classes or 9 credits within her first 2 semesters.
I told her to ask for the reason for this conditional acceptance. She's afraid, the grad school my rescind her acceptance completely. I told her that is very unlikely, if she asks nicely.

Normally, I would have left this alone, but I believe the added pressure may affect her performance.

I was accepted conditionally to one of my schools because they wanted me to do a math prep program (what, my high school calc class doesn't count?! Jerks!) I don't see why she's afraid to ask, as I don't see them rescinding admissions over something so silly.

The conditional admission thing is fairly common and no big deal. Every person I've known who was conditionally admitted did fine the first year. And grades below a B are bad for any Graduate Student.

Actually, now that I think about it, conditional acceptance is the norm for anyone who's coming straight from undergrad. Why? Because the grad school will need proof of a bachelor's ASAP.

Well, my friend finally got an answer in writing, but it's weird. The reason she was given for conditional acceptance was because her gpa was below a 3.0 or she had low TOEFL scores. I have no idea about the TOEFL since she's American and she has a GPA higher than a 3.0.

I thought it might be a major GPA thing, even though we don't have major gpa's recorded on our official transcripts. But how would a foreign school, calculate a major gpa?

She's still is taking it personally and is no longer willing to fight her department, even though I told her to try to talk with the head of the department directly and not just the secretary.

I agree with the departmental reasons, but she had a gpa higher than a 3.0 and she didn't have to take the toefl test. Someone made a mistake. She just doesn't like the "we're watching you feeling".

I was offered conditional acceptance to one of my Computer Science programs... I believe they were mostly worried because I came from a school that didn't officially have a computer science major at the time.

I didn't persue that opportunity, however, because I had several full acceptances with full tuition and assistanceships offered.
