confused about SHIP!

So I'm a bit confused about this whole insurance waiving process.. I have blue shield, met life, etc., all kinds of different insurance for different things. How can I waive my SHIP fee?

I will attemp to help you, but be aware, I didn't waive SHIP last year or this year. :)

Fill out the online waiver:
- Go to http://www.studenthealth.ucla.edu/root/os-main.html
- Click the second category on this page (Logon to Online Services 2006-2007 - Category 2)
- Sign in with BOL or UID
- In the dropdown menu select "SHIP 2006-2007" and click Proceed
- On the next page, in the dropdown menu select "2006-2007" and click Proceed
- Select "Waive blah blah blah"
- Fill out the next pages and you should be good. :D

If you have any other questions, please feel free to ask!

Like i've been telling everyone, if you waive SHIP, be sure to waive before Sept 20th, for the entire year and to get all THREE confirmation numbers.

You use your major medical provider. So if blue shield is what you use when you go to the doctor, use that one.

tomorrow's the 20th, so i'm asking this question (a little) late, but these are some questions i hope will be answered!

My plan has a lifetime benefit of at least $100,000 Yes No
Why does it ask for that? And why does it not let you go on if you dn't have a lifetime benefit?

Facility within 25 miles of UCLA where your plan will provide full care:__

do you write the dr.'s name or address..there is only so little space

thanks

not sure why they impose those specific requirements.

but about the facility, i just put UCLA Medical Center.

same, i put UCLA Medical Center.

how much is SHIP, anyways? Does it show up in URSA/BAR account? I didn't see a noticable difference after I waivered SHIP.

SHIP is billed on BAR quarterly at the rate of $207.

The stipulations are there to ensure that your insurance is sufficient to pay medical costs should you need them.

if we don't waive it this quarter, can we do so next quarter?

so all of ya'll have lifetime benefits up to 100 Gs?

yeah.

well, apparently.

You can waive it next quarter. I'm pretty sure.

According to the same site, US-based HMO plans satisfy those stipulations.

Yeah I think they're just looking for coverage that's equal to what they offer.

It's pretty standard unless you only have catastrophic or something.
