Consent for Medical Treatment?

<p>My child will not yet be 18 upon entering college. Are there any special authorizations or forms that I should sign to give consent to provide medical treatment?</p>

<p>Yes, you will need to sign a form giving consent. This will be included with the stack of forms the college sends over the summer (move-in information, physicial examination form, etc.).</p>

<p>While you’re at it, get the forms for your child to authorize you to see financial, academic & medical information about him. You possibly may also need him to re-sign after he turns 18. It can be tough on parents when their kids haven’t signed so the parents don’t get info but are still on the hook for bills but can’t communicate to be sure bills & credit are correct.</p>

<p>We found that each school differs. And that you can’t count on the school to send you the forms. </p>

<p>I recommend calling all the places that you anticipate consent may be needed, medical (for emergency, flu shot, ask if there could be other), housing (probably needs adult consent on contract), dining, banking, orientation office (will there be an orientation adventure -rock climbing, kayaking - that requires consent). Then maybe an additional call to student affairs office to see if you have forgotten anything.</p>
