<p>Yes.</p>
<ul>
<li>It does not need to be super fancy.
It should include</li>
<li>your name</li>
<li>telephone number</li>
<li>colleges you're applying to</li>
<li>a list (with a brief description) of what activities you do</li>
<li>list your test scores</li>
<li>tell them why you're applying to these schools, and what you hope to get out of your education there</li>
<li>any awards/honors you've received</li>
<li>list of due dates for recommendations</li>
</ul>
<p>Also attach
- stamped and addressed envelopes
- one folder with your name on it so that your teachers have a nice place to store everything so nothing gets lost</p>
<p>Also, ask if they'd like you to meet during lunch or something to talk about anything about this further.</p>
